Please contact the office at 704-786-4503, extension 3, to let us know of your interest in services. Leave a message with the best number and times to reach you if we are unavailable when you call.

Have your insurance card ready when you call. We can let you know if we are enrolled with your particular insurance company. If we are not enrolled, or you are not sure of your mental health benefits, we can contact your insurance company for you to find out your status (what your co-pay or deductible is, for example).

The next step is to complete the the forms below and send them to our office.
